What is the Hospital to Recovery Program?
The Hospital to Recovery Program is a free, 12-week, Peer-led support initiative with a focus on non-clinical, practical support. Our Peer Support of Suicide Prevention and Postvention Services, Jade, leads this psychosocial program from Lifeline Mid Coast.
Eligible participants are those who have experienced a suicide attempt, struggle with suicidal thoughts or self-harm.
This Lifeline psychosocial program is accredited by Suicide Prevention Australia QIP. It is designed for the purpose of supporting individuals to regain stability and reintegrate into daily life in their community, in collaboration with other mental health professionals.
Program support areas may include:
Daily living Support: Medication management, crisis management and safety planning.
Referrals and Advocacy: Advocacy for ongoing services, connection to Lifeline support groups and assistance with applications and referrals for ongoing support.
Engagement and empowerment: Wellbeing checks, appointment management and transport.
A transition plan with be created as participants near the end of program completion, to ensure all necessary referrals are completed and identify support services for continued care if needed.
Referrals can be made by a clinician, carer or other support service provider. Self- referrals are also welcome. For more information or to submit a referral, please contact [email protected]
